Never Event Occurs With Epidural Painkiller Mistake

An epidural mistake classed as a ‘never event’ took place at Croydon University Hospital. A ‘never event’ is described in the medical world as a mistake so serious and preventable that is should never happen. In this instance, a patient was given painkillers through an intravenous drip as opposed to through a spinal injection.

Fortunately, no harm came to the patient on this occasion. However, the incident was classified so severely because of the risk it carries. However, since the mistake took place, the national guidelines have reduced the severity of the threat and is no longer classified as a ‘never event’.

During an epidural situation, time is critical, and accuracy is essential for the safety of the mother and child. In this instance, the medical line delivering the painkilling drugs was connected incorrectly. However, staff quickly spotted the mistake and rectified with the correct solution so that the patient, luckily, did not come to any harm.

As a result of this situation, the Croydon Health Services NHS Trust created new guidelines and reviewed the performance to ensure that cases like these, do not happen again as the consequences can be severe.
